If you have been involved in a motor vehicle accident, you have to fill out a Massachusetts motor vehicle crash operators report. you may download these here http://www.mass.gov/rmv/forms/accident.htm or pick them up at any police station or Mass RMV.
You will need 3 copies of this form, 1 for the local police department where the ACCIDENT OCCURED, 1 for the Mass RMV, 1 for your insurance company. If you have been involved in an accident in the Town of Stoneham on Lynnfells parkway, route 93, Woodland Road or South St, you must file your report with the
Massachusetts State Police, as those roads are under State Police jurisdiction. A list of State Police locations can be found here . You have 5 business days after the accident to complete and file these forms.
